Prairie Lakes Brown Clinic has been recognized by the Patient Centered Medical Home (PCMH) program for the past 8 years and again for 2024! 

Organizations that are recognized by the PCMH program are found to have a better patient experience, better managed chronic conditions, improved care coordination, improved staff satisfaction, lower health care costs, and align with state and federal initiatives. This is a yearly submission of reports/data and recognition of Prairie Lakes Brown Clinic. The PCMH model has positive effects on quality care and holds our organization accountable to maintaining best practice and evidence-based guidelines with the patient at the forefront.  

To qualify for PCMH recognition, the practice must meet a minimum number of requirements in six areas including: 

  • Patient-Centered Access
  • Team-Based Care
  • Population Health Management
  • Care Management 
  • Care Coordination and Care Transitions 
  • Performance Measurement and Quality Improvement 

Practices must submit supporting data and documentation to fulfill the requirements of each quality category.

  • Patients who receive their care in PCMH organizations tend to receive preventative services and screenings at higher rates to help keep them healthy, reduce the risk of disease, and improve quality of life. Clinics with a PCMH model and focus provide exemplary communication to their patients and their families/caregivers along with a priority to optimizing patient access to clinicians and care coordination. Research shows that patients receiving care with a PCMH provider have better managed chronic conditions and a superior patient experience. 

What does this recognition mean to staff? 

  • Organizations with a Patient Centered Medical Home model have been shown to result in higher staff satisfaction as they are able to streamline processes and allow staff to work in their highest scope of practice. Working within a practice that is upheld to PCMH standards ensures the highest level of care, strong infrastructure, and robust quality improvement initiatives.
