Patient Centered Medical Home

A patient-centered medical home is a system of care in which a team of health professionals work together to provide all of your healthcare needs. We use technology such as electronic medical records to communicate and coordinate your care and provide the best possible outcomes for you. 

You, the patient, are the most important part of a patient-centered medical home.
When you take an active role in your health and work closely with us, you can be sure that you’re getting the care you need.

Who is on the patient-centered medical home care team?

Your primary care provider (the doctor you choose) leads your care team, which may include specialized doctors, physician assistants, nurse practitioners, non-physician clinicians such as pharmacists, dieticians, physical therapists, lab, x-ray technicians, nurses, medical assistants and other support staff. Our team acts as “coaches” who help you get healthy, stay healthy, and get the care and services that are right for you. You, of course, are at the center of your care team.

Your Care Team Will:

1. Get to know you

  • Learn about you, your family, your life situation and preferences. We will update your records every time you seek care and suggest treatments that make sense for you.
  • Listen to your questions and feelings and treat you as a full partner in your care.

2. Communicate with you

  • Explain your health situation clearly and make sure you know all of your options for care.
  • Give you time to ask questions and answer them in a way you understand.
  • Help you make the best decisions for your care.

3. Support you

  • Help you set goals for your care and help you meet these goals every step of the way.
  • Give you information about classes, support groups, or other services that can help you learn more about your condition and stay healthy.

In our patient-centered medical home at Prairie Lakes Brown Clinic, we:

Are available when you need us. You can communicate with us easily and efficiently and get appointments quickly. Our office is open from 8:00 am-5:00 pm with walk-in services from 7:30 am-9:00 am & 5:30 pm-7:30 pm Monday- Friday at our Northridge location. Prairie Lakes Brown Clinic provides same day appointments for routine care (regularly scheduled primary care) and for urgent (for those times when a patient wants or needs to be seen by a provider quickly during regular business hours). When our clinic is closed, the main telephone will connect you to the on-call doctor who will help you. You can also contact us through our website.

Know you and your health history. We know about your personal or family situation and can suggest treatment options that make sense for you.

Help you understand your condition(s). We explain your options and help you make decisions about your care. We use evidence-based guidelines (the best methods of treating you) to take home that are easy to read and understand.

Help you coordinate your health care. Even if we are not the ones giving you the care, we will help you find hospitals, specialists, get appointments, and make sure specialists have the information they need to care for you. If you need behavioral health and are referred to a behavioral health organization for diagnosis and treatment plan and then referred back to your doctor for follow up care.

Use technology. Electronic medical records and share records help prevent medical errors and make sure that we are always on the same page. This technology also helps us remind you when you are due for preventive health visits or care (your annual physical exam, mammogram, colonoscopy, diabetic care and immunizations).

Equal access. Regardless of the source of payment, we will provide financial counseling. If you do not have insurance, we will provide coverage resources for you.

New patient. We will assist you in transferring your records to our clinic by providing request forms at the front desk and will help coordinate this process for you. We will provide you with a packet of information about us.

Accountable Care Organization

Prairie Lakes Brown Clinic is participating in OnBelay, an Accountable Care Organization (ACO). An ACO is a group of doctors, hospitals, and/or other health care providers that work together to improve the quality and experience of care you receive. 

Prairie Lakes Brown Clinic's participation in an Accountable Care Organization doesn't limit your choice of health care providers. Your Medicare benefits are not changing. You still have the right to visit any doctor, hospital, or other provider that accepts Medicare at any time, just like you do now

How do ACOs work?

An ACO isn't a Medicare Advantage plan which is an "all in one" alternative to Original Medicare, offered by private companies approved by Medicare. An ACO isn't an HMO plan, or an insurance plan of any kind.  

ACOs have agreements with Medicare to be financially accountable for the quality, cost, and experience of care you receive.

Coordinated care can avoid wasted time and costs for repeated tests and unneeded appointments. It may make it easier to spot potential problems before they become more serious - like drug interactions that can happen if one doctor isn't aware of what another has prescribed. 

ACOs may use electronic health records, case managers, and electronic prescriptions to help you stay healthy. Some ACOs have special programs to encourage you to have a primary care visit or use their care management team. Participation in these programs is optional. 

How can I Make the Most of Getting Care From an ACO?

Ask your clinician if they have a secure online portal that gives you 24-hour access to your personal health information, including lab results and provider recommendations. This will help you make informed decisions about your health care, track your treatment, and monitor your health outcomes.

As a Medicare beneficiary, you can choose or change your primary clinician or "main doctor" at any time. Your primary clinician is the health care provider that you believe is responsible for coordinating your overall care. If you choose a primary clinician, that clinician may have more tools or services to help with your care.

FORMS

Minor Consent Forms

Prairie Lakes Brown Clinic is required to protect the rights of our patients and ensure the safety of minors. Any new patient under the age of 18 must be seen in the presence of their parent and/or legal guardian. It is important that the parent/legal guardian is aware of the minor's condition or conditions, and everyone understands and follows through with the recommended treatment plan. Parents and/or legal guardians will be asked to provide identification via a valid photo ID.

An established patient (a patient known to the practice and seen within the past 12 months) under the age of 18 may be brought in for a specific visit or timeframe by a non-custodial adult if written permission is given by the parent or legal guardian, and that adult shows a valid photo ID. The parent or legal guardian must leave a number where they can be reached on the date of service.

A signed and witnessed consent form from the parent/legal guardian may be signed prior to the visit, allowing the minor patient to be seen without the parent/legal guardian present.

There are certain types of minors authorized to consent because of their status. Some examples are, but not limited to, married minors, minors on active duty with the U.S. Armed Forces, and minors emancipated by a court order.

Sports Physical Parent/Guardian Permit & History Forms

All high school athletes need to schedule a sports physical with their provider. Prior to the exam, the forms must be completed and signed by the student's guardian.

POLICIES

Financial Policy

Prairie Lakes Brown Clinic is committed to providing you with the best possible medical care; if you have special needs, we are here to work with you. The following information is provided to avoid any misunderstanding or disagreement concerning payment for professional services.

  • The total patient balance due, including any co-payment or co-insurance, is required to be paid at the time services are provided. For your convenience, we accept cash, checks, Visa, MasterCard, Discover and American Express.
  • Our office participates with a variety of insurance plans. It is your responsibility to:
    • Bring your insurance card to every visit.
    • Be prepared to pay your co-payment at each visit. Payment can be made by cash, check,or credit card.
    • For medical care not covered under insurance, the estimated payment in full is due at the time of the visit.
  • If you are unable to pay for necessary medical care, you may be eligible to participate in a payment plan. It is your responsibility to inform us of this prior to your visit. We will be pleased to refer you to a Prairie Lakes Brown Clinic Financial Counselor prior to scheduling you for a visit.
  • Referrals: It is your responsibility to bring any required referrals for treatment at, or prior to the visit. If you do not have the referral, your visit may be rescheduled or you may be held financially responsible.
  • If the patient is a minor (younger than 18 years) and is unaccompanied by a parent or guardian, we must have a written release/permission from the parent or guardian. The parent, guardian or unaccompanied minor is responsible for any payment due at the time of service, bringing the necessary referrals and the insurance card.
  • If you have questions about your insurance, we are happy to help you. Specific coverage issues, however, should be directed to your insurance company member services department (number is on the insurance card).

Prairie Lakes Brown Clinic firmly believes that a good patient/physician relationship is based upon understanding and good communications. Questions about financial arrangements should be asked prior to services provided.

The HIPAA Privacy Rule requires health plans and covered health care providers to develop and distribute a notice that provides a clear, user friendly explanation of individuals rights with respect to their personal health information and the privacy practices of health plans and health care providers. 3D Mammograms

Prairie Lakes Brown Clinic is proud to offer you 3D mammography – the newest technology in breast cancer detection. We know breast cancer is most treatable when it’s caught early. 3D technology produces sharper images that provide detail never before possible. It also means fewer false positives, which equals fewer callbacks and greater peace of mind for you.

Screening with 3D Mammography

3D mammography allows doctors to look at your breast tissue one layer at a time. While digital mammography is still one of the best technologies available, it is only 2-dimensional, producing a flat image of the breast. Overlapping tissue in a flat image can make it difficult to see small breast cancers or may make normal tissue appear abnormal.

3D mammography takes multiple pictures of the breast tissue at various angles. These slices are then put together to create a 3-dimensional image – much like the pages of a book. Page by page doctors can see more than when looking at the “book” as a whole.

3D mammography is done in conjunction with a standard digital mammogram. There is no additional compression needed and uses very low X-ray energy, just about the same amount as a traditional mammogram.
